Custom longarm quilting designs can include moderate stitch in the ditch, special block patterns, stencils, connecting designs, detailed border treatments, multiple feathers designs and some ruler work. All the designs will work together to create a uniform density of quilting. Typically, this level of quilting falls between 5 cents per square inch to 10 cents per square inch.
As far as "how" your quilt is quilted we can go with free motion quilting, or with robotics which would entail utilizing digitized quilting designs. There is a wide array to choose from, and they are jaw dropping gorgeous and available in "collections" such as those shown on the following websites:
What a beautiful appliqué quilt. Quilted with meandering feathers in the green background, stippling in the white block backgrounds, and everything stitched in the ditch.

This is a monster (100" x 120") King size quilt. That was sheer joy to quilt. It has a combination of swirl and geometric quilting designs. With an extra grey border around the outside. (Yes, that's me in the corner.)
This lap size quilt (60" x 74") is just gorgeous. With a cream background and stars in a variety of batik fabrics. The quilting is a combination of swirls and geometric lines.
This little topper is a great example of how curves and straight lines go beautifully together. And is even further enhanced by pebbling in the fill.

I love winter. Sparkling snowflakes, skiing, swooshing downhill, blustery wind. These are the things I tried to capture with my quilting.
This little topper is filled with lots of dense detail quilting. That's the main reason this quilt is expensive. All the applique is outlined too.
I ditched all the blocks and sashings and outlined all the pieces within each block. Then McTavished the background. Which added awesome texture.
